Record requests typically take up to seven days to be processed. There is no cost to businesses requesting records but there is a $1.00 per page fee for individuals to receive copies.
If you would like to receive copies of your records you must fill out this form and submit to Department of Human Services.

Records may be picked up on the top floor of the Annex building and you must bring photo ID.
