To request records, please call 608-847-2400 and ask to speak to the Intake/Records Clerk to begin the process. Client Rights and Grievance
If you receive services for mental health, substance use, or a developmental disability and have a complaint, you have the right to file a grievance. Open links below for more information or to complete a Grievance Form. Completed Grievance Form can be printed and mailed to address stated on form, or emailed to dhs2@juneaucountywi.gov |
Prior to receiving services (unless in a crisis situation), any consumer of JCDHS is to meet with our billing department to complete all billing paperwork. See Billing Procedures, Uniform Fees, Liabilities, and Collections for more information.
