Aging and Disability Resource Specialists (ADRC Specialists)

ADRC Specialists are available by phone and appointment in our office or your home. We listen to you and your unique situation, focusing on identifying needs, preferences and support, both personal and financial. Staff explore and link you to programs and potential funding sources so you can make decisions that are right for you.
The ADRC is the entry point for accessing publicly funded long term care programs. These programs are designed to assist individuals to remain in their homes, retain independence and delay long term care nursing home placement.

Options Counseling is a person-centered, decision support service that empowers older adults, adults with disabilities and their families/caregivers to make informed decisions about current or future long- term care choices.  ADRC Resource Specialists provide reliable information about the choices you have when making decisions about where to live, what kind of help you need, where to receive that care and help and how to pay for it.  We offer one-on-one consultation in your home or in one of our local offices to help you think through the pros and cons of the various options in light of your situation, values, resources and preferences.

Tell me more about Enrolling in Publicly Funded Long Term Care Programs


ADRC will provide both options counseling and enrollment counseling for eligible individuals wanting to enroll into a long term care program. Eligibility is determined by:
  • Age 18 or older
  • Frail elder (age 65+), developmentally / intellectually disabled, or physically disabled
  • Functionally qualify based on the Long Term Care Functional Screen
  • Financially qualify for Medicaid

Family Care
The Family Care program offers a wide range of services that meet the needs of customers wherever they may live. A person receives case management provided by a care manager and a registered nurse.

IRIS (Include, Respect, I Self-Direct)
IRIS allows for self directed services and puts the individual in charge of his/her own long term care. An individual creates plan outlining long term care supports and services with an assigned monthly budget. Individuals are able to hire their own service workers directly or may utilize an agency or community provider. Individuals enrolled in IRIS work with an IRIS Consultant and the IRIS Financial Service Agency which pays for the services participants have authorized.
